Yes, bring it if you already have it and we will complete and sign it during the visit. If you do not have it, we still perform the exam and give you the results in writing.

What a sports physical checks

It is not a formality. What it is mostly looking for is anything that could cause a problem during exertion:

- **Family history of cardiac problems**, which is the single most important question on the form - Blood pressure, pulse and heart auscultation - Vision and hearing - Spine, joints and previous injuries that never healed properly - Weight, height and development

If family history or the exam raises a cardiac question, we tell you and help direct the referral before clearing the sport.
